Carlo Gugino was born on 29 August 1971 in Sarasota, Florida, United States. Her father is Carl Gugino, an orthodontist of Italian descent and mother of English-Irish descent. When she was 2 years old her parents separated . She travelled between her father and half-brother Carl Jr.’s home in Sarasota, and her Paradise, California, home, to which her mother moved her when she was four. When she was teenager , she worked as a fashion model and took acting classes.

She was nominated for an Outer Critics Circle Award for Outstanding Actress in a Play for her portrayal in Desire Under the Elms in April of that year. Later that year, she starred as a pornographic actress in the comedy film Women in Trouble, which was followed by a sequel, Elektra Luxx, in 2010, based on her role. Gugino starred with Abbie Cornish and Emily Browning as Madame Vera Gorsky in Zack Snyder’s action-adventure flick Sucker Punch in 2011. Gugino sang a duet with co-star Oscar Isaac that was featured in the film’s end credits and soundtrack. She also appeared as Abby Rhodes, Hank Moody’s attorney and love interest, in the fourth season of Californication. Gugino starred as Susan Berg, a Washington, D.C.-based investigative reporter, in the USA Network miniseries Political Animals in mid-2012. She has a lead part in the catastrophe blockbuster San Andreas in 2015, opposite Dwayne Johnson for the second time.
